Where can I get an intake form?

Q: I would like to present my (or my client's) case to Mr. Tooby, or ask him some questions regarding a criminal immigration case. A: Before we can help you, we need to gather some basic information about you (or your client's) current case and personal history. Please fill out an intake form and submit it, along with any relevant documents, to our office. You will be contacted in approximately two weeks. Q: Where can I get an intake form? I am not able to access the Premium Resources. It's not accepting
my password and the link to have the password sent is not working. A: This problem can occur when your browser is not set to accept cookies. If you are using Internet Explorer go into Internet Options under the Tools menu and select the privacy tab. Select the Advanced button and set it to accept cookies. Q: I signed up for the premium membership on a monthly basis, but I am getting emails saying I'm not making my payments. A: If you have set your account on automatic renewal it is most likely that we were unable to charge your credit card on file due to expiration or a change of address. Please click here to manually renew your payment method. Q: I signed up for the premium membership, paid and received my verification email, but when I put the username and password in I get an error message and am not allowed in.A: If you are receiving the "FAILED LOGIN" message or "SORRY, LOGIN WAS NOT FOUND" message after clicking on the verify email link, check that the ENTIRE verify link is a hyperlink Sometimes certain kinds of webmail only display a portion of the verify email link as a hyperlink. If this is the case select the ENTIRE link and paste in into your browser's address line. After doing this you will be brought to a page stating "Your account is now verified". Press the "LOGIN HERE" button then enter your login and password.
